We offer a wide range of mental health and social services at Thomas Chittenden Health Center, many with no or minimal cost to our patients. Our Clinical Social Worker provides acute intervention as well as short and medium term counseling in our office 5 days a week. Our Psychiatric Nurse Practitioner provides psychiatric consultation, evaluation and treatment, including management of psychiatric medications, health counseling and psychotherapy.

Our Nurse and Social Work Care Coordinators provide more intensive support for patients who struggle with complex medical or social needs. They also provide transition of care services to ensure smooth transition and timely follow up after discharge from a hospital or rehabilitation facility.
