Primary Care Provider

We are seeking a dedicated and compassionate Primary Care Provider (MD, DO, NP, or PA) to join our dynamic team. The ideal candidate will provide comprehensive outpatient care, build long-term relationships with patients, and contribute to our mission of delivering high-quality healthcare in a personalized setting. This can be either a full-time or part-time position.

Responsibilities include:

• Providing preventive, acute, and chronic care management for patients of all ages.
• Collaborate with a multidisciplinary team to ensure continuity of care.
• Maintain accurate and timely documentation in our EMR system.
• Participate in quality improvement initiatives and practice development; and foster a welcoming and respectful environment for patients and staff.
• Must be licensed (or eligible for licensure) in the state of Vermont.
• Have strong communication and interpersonal skills.
• Have a commitment to patient-centered care and community health.
• Experience in outpatient primary care preferred.

Nurse Care Coordinator

Mid-sized, busy family practice is seeking a caring, compassionate, responsible nurse to serve as a Nurse Care Coordinator.

• This role includes developing and coordinating care plans, supporting patients and families, and conducting transition-of-care phone calls to ensure smooth discharges from hospitals/rehab.
• Must have excellent communication and organizational skills.
• Valid Vermont nursing license required.
